Lash lIFT Consent

DESCRIPTION OF TREATMENT

Using a small curling rod and lifting solution your natural lashes are curled from base to tip. If desired, tint may also be applied to alter the color of the lashes. The treatment requires maintaining your eyes closed and covered for the duration of the approximately 60-minute procedure.

POTENTIAL RISKS AND SIDE EFFECTS

Although your technician uses proper technique, instruments, adhesive, tape, cleansers, eye gel pads, and removers, your eyes may become temporarily irritated or in rare cases, require a physician’s care. If you experiences any of the following conditions in association with your lash lift procedure, you should contact your technician and consult a physician at your own expense.

  • Eye irritation

  • Eye pain

  • Eye itching

  • Blurred vision

  • Infection in the eye area

Notification of relevant conditions

If you have any of the following conditions, it is imperative that you inform your lash technician as it may affect your eligibility for this procedure:

  • If you have had ocular surgeries, including blepharoplasty (eyelid lift) or LASIK, you must wait a minimum of 6-12 months before receiving this treatment

  • Current use of contact lenses which I agree to remove during the procedure

  • Current use of eye drops of any kind

  • Current allergies to anything that could cause my eyes to water or blink excessively

  • History of recurrent eye or tear duct infections

  • History of dry eyes or Sjogren’s Syndrome

  • Other relevant conditions that may affect the procedure

Pre-care

Please follow the guidelines below to prepare for your treatment and ensure the best possible outcome:

  • Avoid caffeine the day of the appointment to limit eye movement during treatment

  • If you have eyelash extensions, they need to be removed at least one day before your appointment

  • Do not use waterproof mascara or curl your lashes the day of your treatment

  • Remove any eye make-up the day before your appointment

  • If you need to wash your hair, it is best to do it before your appointment to avoid getting your lashes wet for at least 24 hours after treatment

aFtercare

To ensure your lash lift lasts as long as possible, it is important to adhere to the following aftercare instructions. Failure to follow these instructions may reduce the longevity of your lash curl and the overall performance of your procedure:

  • Refrain from getting your freshly lifted lashes wet for 24 hours

  • Use a gentle foaming cleanser to clean your lashes (after 24 hours)

  • Use an oil-free eyelash enhancing serum daily to help sculpt, position, and protect lashes

  • Avoid spas and saunas

  • Avoid rubbing as much as possible with hands, wipes, etc.

  • (After 24 hours) Wet and brush lashes with spoolie daily

  • Re-book every 5+ weeks to maintain you gorgeous lashes

Client CONSENT

I release my technician from all liability associated with the procedure, which is performed with the utmost attention to safety and proper application using tools and products that the technician has been professionally trained to use. I understand that additional conditions could occur or be discovered during the procedure which could affect my ability to tolerate the procedure as well as the outcome.

This agreement will remain in effect for this procedure and all future follow-ups conducted by the technician. I have read and understand that this consent agreement is legal and binding and fully understand all information herein. I am over 18 years of age and consent to the agreement and to the lash lift procedure.
