Waxing + Sugaring Consent

 

DESCRIPTION OF TREATMENT

Hair is removed to the root using hot wax or sugar paste and removal strips, depending on the hair removal procedure selected.

POTENTIAL RISKS AND SIDE EFFECTS

Possible side effects include soreness, small bumps and redness. Side effects are more common if you have not undergone wax/sugar hair removal before, but are temporary and generally fade within 24-48 hours. If you experience persistent redness or irritation, contact your hair removal specialist.

Notification of relevant conditions

If you have any of the following conditions, it is imperative that you inform your hair removal specialist as it may affect your eligibility for this procedure:

  • Used any Alpha Hydroxy Acid (AHA) or glycolic products in the past 48-72 hours

  • Use Retin-A, Renova or Accutane (an oral form of Retin-A)

  • Use any other skin thinning products and/or drugs

  • Are exposed to the sun on a daily basis or will be spending more time in the sun soon

  • Use a tanning bed

  • Are diabetic

  • Will be on your menstrual cycle within two days of your appointment

  • Have any open skin lesions or active herpes outbreak (cold sore or genital)

  • Are currently taking medications or use skin products that may affect the procedure

  • Are currently being or have ever been treated for cancer

  • Are being treated for an illness/condition that may affect the procedure

  • Have any allergies that may affect the procedure

Client CONSENT

I agree that this constitutes full disclosure, and that it supersedes any previous verbal or written disclosures. I certify that I have read, and fully understand the above paragraphs and that I have had sufficient opportunity for discussion and to have any questions answered. I understand the procedure and accept the risks.

I have given an accurate account of all conditions that may affect the procedure. I understand my hair removal specialist will take every precaution to minimize or eliminate negative reactions and release IKAL Spa + Face Salon from all liabilities associated with this procedure and all future hair removal procedures that I undergo at IKAL.
